Photograph your dispensing label Find the dispensing label (the pharmacy sticker on your box) and recent order records. Do not upload them here. Write down your treatment history Note your last dose, any breaks, side effects and other medicines. Check the new pharmacy Check the pharmacy is registered and find its clinical contact details. Ask which records it accepts Confirm its policy on transfers and maintenance doses (your long-term dose), and which records it takes. Compare full and repeat costs Add delivery, compulsory fees and the price after any offer ends. Pick a pharmacy you have not used. Complete the consultation honestly Say you are already on treatment. Answer identity, measurement and health questions accurately. Read the written treatment plan Check the instructions. Ask about anything unexpected before you use the medicine. Confirm delivery and any gap plan Approval and dispatch are separate stages. Start at least a week before your next dose is due. Cancel the old subscription in writing Check the cut-off and any order already processed. Keep the cancellation confirmation. Print checklist Reset Your ticks last only while this page stays open.
