The prescriber decides. A new month or a new pharmacy does not mean a higher dose. Asking for a review never gives you permission to change your dose yourself. ## Your three dose options before the next order Before each order you can ask to go up, stay the same or go down. More medicine is not always better. Tell your prescriber (the doctor, nurse or pharmacist who approves your medicine) how you are getting on and what you want. They weigh your answers against the rules for your medicine. Questions for your prescriber. This is no personal dose advice. You do not need to claim worse side effects, exaggerate weight changes or tick a misleading answer on a form to get a different plan. ## Mounjaro dose increases: the UK schedule Under the UK prescribing information, adults start on 2.5 mg a week. The dose goes up to 5 mg after four weeks. Any further increase is 2.5 mg at a time, after at least four weeks on the current dose. The recommended maintenance doses for adults are 5, 10 and 15 mg. The top dose is 15 mg a week. Read the current Mounjaro prescribing schedule (https://www.medicines.org.uk/emc/product/15481/smpc). The 2.5 mg starting dose is not a standard maintenance dose. Nor are 7.5 mg and 12.5 mg. If your plan is different, ask the prescriber why and how your reviews will work. A seller offering a strength does not show the strength suits you. ## Wegovy injection doses: steps to 2.4 mg and 7.2 mg UK guidance builds Wegovy injections up over 16 weeks. You go from 0.25 mg to 0.5, 1 and 1.7 mg, then on to 2.4 mg a week. Current guidance also allows 7.2 mg for some adults with obesity when needed, after at least four weeks on 2.4 mg. Not everyone has to move up to 7.2 mg. Bad digestive side effects can be a reason to delay an increase or go back to the previous dose until they ease. Never apply an injection strength or step to a tablet. Use the leaflet for the exact medicine you receive and ask the prescriber to confirm your plan. ## Can I stay on my current Mounjaro or Wegovy dose? Yes, you can ask. Ask for a review before the next prescription is made up. You do not have to end up on the highest dose. Maintenance means a plan you agree with your prescriber. Picking the cheapest strength is no plan. Ask two separate questions: “Can I delay the next increase?” and “Is this a good longer-term dose for me?” A good answer gives the reason, how long the decision lasts and when to check back. If the repeat-order screen seems to offer only an increase, message the clinical team. Do not guess which box to tick. Save their written reply with your treatment records. ## Can I ask to go down a dose? Yes. Ask why a lower dose would or would not suit you. Tirzepatide is the medicine in Mounjaro. If you get side effects while the dose is going up, NICE tells clinicians to wait until symptoms settle or become bearable. Only then should the dose go up again. If symptoms do not settle, NICE says to consider a 2.5 mg reduction. This is advice for a clinical problem. NICE does not say you may drop a dose whenever you prefer. Tell the prescriber what worries you. Examples are symptoms, ongoing weight loss, eating enough or cost. Ask which option deals with your worry. A lower number does not fix everything. If you go down, get clear instructions for your next order. A lower strength and longer gaps between doses are two different changes. Never swap a dose review for your own schedule, part-pen doses or a sum copied from social media. ## Keeping your dose when you switch pharmacy each month Many people switch pharmacy every month (https://pharmacyhopper.co.uk/guides/switch-pharmacy-every-month/) to pay the new-customer price. You can usually ask the new pharmacy to keep your dose or move you up to the next one. The new prescriber has to agree. Give the new pharmacy: - Your current prescription , with the medicine and strength. - Your real dose dates , and how long you have been on this strength. - Any gaps or missed doses. A break can change what is safe. - The review you want : stay, go down or go up. Your current plan gives them context. The new prescriber still has to check the plan. Ask before you pay: “I take [medicine and strength]. I would like a review of staying on this dose / reducing / increasing. Can your clinical team check this? What records do you need? What happens to my order if the strength I ask for is not suitable?” A low price is no use for a strength you will not be prescribed. Sort out the dose first. Then compare prices for the same approved order and the support you need. Keep the money question clear: “Which suitable options can I afford?” Then compare the total for each one, including repeat prices and any minimum term. They do not choose your dose. ## Five details to take to your dose review - Your medicine: its form (such as pen or tablet), current strength and the date you started this strength. - Recent dose dates , including any you missed. - Your progress and the worries you want answered. - Side effects , other medicines or health changes. - Your preferred option , any money worries and your next order date. Leave with the exact strength, next dose date, order details and review date in writing. If the new label (the pharmacy sticker on your box) does not match the plan, ask the dispensing pharmacy before you use the medicine. ## Symptoms needing urgent help instead of a dose review Get urgent medical help for severe stomach pain which will not go away, especially pain spreading to your back. Vomiting or diarrhoea which keeps going can make you dehydrated. Do not wait for a reply about an order. Do not assume a lower dose will fix a serious symptom. No. Under UK prescribing information, Mounjaro starts at 2.5 mg a week and goes up to 5 mg after four weeks. Further 2.5 mg increases happen only when needed, after at least four weeks on the current dose. The top dose is 15 mg a week. What are the recommended Mounjaro maintenance doses? UK prescribing information lists 5, 10 and 15 mg as the recommended Mounjaro maintenance doses for adults. The 2.5 mg starting dose, 7.5 mg and 12.5 mg are not on the list. If your plan is different, ask your prescriber to explain why. Can I ask to reduce my Mounjaro or Wegovy dose because of side effects? Yes.
